No. You receive the nozzle, two water bottle adapters, one shower adapter and a storage cover. Supply your own compatible bottle or shower hose.
Is this a travel bidet or an anal douche?
It is an anal douche attachment for internal cleansing. A travel bidet sprays the outside of the body; the two products have different intended uses.
How is it different from an enema bulb?
